HPV DNA screening and vaccination strategies in Tunisia

A cost-effectiveness analysis comparing HPV DNA-based cervical screening strategies against the existing Pap-smear approach, both layered on top of Tunisia's 2025 school-based HPV vaccination program for girls aged 11-12.

Key findings

  • All screening strategies combined with vaccination substantially reduced cervical cancer cases, with cumulative-case decreases of 41% to 59% between 2025 and 2090.
  • The most intensive strategy (HPV DNA testing every 5 years for women aged 35-60) achieved the largest reduction but also the highest cost; maintaining current Pap-smear screening alongside vaccination was the most cost-effective option at today's test prices.
  • If the HPV DNA test price fell to $9, the most frequent testing strategy would become the most cost-effective option, offering both the greatest health benefit and financial viability.
